Required Paperwork for Middle School Sports 2022-23.
The upcoming School Year is going to be GREAT! With that comes the return of the activities and extracurriculars that the school year brings. This includes Athletics!! If your student is planning on playing any sports this school year they will have to have a sports physical signed by a physician after 6/30/2022. This form must be updated yearly.
